I have a really bad fear of dying any hlp please.?

WEll u no that t.v show 100 ways to die? i was siting realxing watching that. Laughing at all the dumb stuff , and convienyent things that happen that make people die. Than it hits me . BOOM! im going to die. what if when i die is just blank i take my last breathe and i die at the age of 15 years old. Than my whole faith of god ad jesus crist faded. What if is just blank, i mean to me that would be worse that rotting in hell it self. PLEASE help. This happened 2 days ago and im afraid to fall asleep, i had 2 anxiety attacks. i just need some 1to talk to honestly. about my religion and stuff so please help for petes sake.
